Well, the only ones that have tho be trialed is Knight to Master. It's not GJO policy to always trial Knights.

Wasn't a trail decided for Salemn because her training threads and such were no longer available? I'm not really sure why they weren't, but a trial was agreed on because there wasn't adequate reference material for the Council to render a decision on her being made a Knight. I think that was the case, a semi special one.

Eldrak: I think a Jedi Knight should not be judged by only the ability to fight. I am of the belief there is much more to being one of such prestige as a Knight than to do what many would consider needless battle. A non violent solution can almost always be found and is how I strive to achieve a resolution to any conflict I myself am presented with.
