What are the requirements to become a sith knight, or are they a case by case issue?

More than likely a case by case issue. Depends on how active said person is, in their training and in other missions and all that.

A few different ways, but probably one that would work out the most. Join a Sith Group. :p Get someone to train you, be loyal, do what they tell ya and all that good stuff. I can tell ya though it will take a bit of time to actually acquire the rank of Sith Knight...

It's normally the master of the siths choice whether or not you get to be promoted, so id join up at a group then you can get your lightsaber and your promotion :p
