Permission Form

Hi, whenever I first registered I was 13 and for some reason I had to do the permission form thing. Well, I am 18 now, and it still has me as underage. Could an admin get rid of this for me please?

Also, if it isn't too much trouble, could I get a name change to "Patrick" please? I feel like I have changed my name more times than I have actually posted here in the past few years.
